To encourage all participants to receive a vaccination to protect against COVID-19, members and their covered eligible dependents (spouse and/or children) are eligible for a vaccination benefit. Members can receive $200 for getting vaccinated and $100 for dependents. You have 6 months from the date of vaccination to submit your application. You may apply for vaccinations received on or after July 1, 2020 and before January 1, 2022 during the 2022A stabilization period (1/1/2022 to 6/30/2022). To apply for this benefit, you will need to complete the online form “COVID 19 Vaccine Subsidy” under “Applications” in SASMI’s member portal and provide a copy of your vaccination card. Receipt of this benefit does not limit a participant’s eligibility for other benefits under the Plan and is not taken into account when calculating severance or HCRA benefits.
